Billing charges for office visits and medical procedures are determined by the severity and complexity of your medical problem. Please do not hesitate to discuss fees with us, if you have any questions.

We appreciate payment at the time of the office visit, when services are rendered. You may use cash, check or credit card. Please ask to speak with the billing secretary if you need to arrange a payment plan. She will be happy to assist you.

An up-to-date, valid insurance card is required at the time of service. Our billing office will submit to all insurance companies that we participate with.

Co-payments must be paid at the time of the visit. You are responsible for all deductibles and non-covered services.

Referrals

Many insurance companies require referral forms for most any surgical or medical consultation, blood work, x-ray, pathology, procedure or hospital stay. Benefit information should be obtained directly from your insurance company. Please direct any questions or complaints regarding your insurance coverage to your insurance company. You are responsible for requesting referrals. We will be happy to facilitate the referral when it is requested; however your insurance company has the final say in authorization of the referral.
